2012 Health Professions Admissions Forms
The following are the forms that you will need to submit for consideration. All students must submit the appropriate Health Professions Program Application (formerly called Admissions Eligibility Forms) for their program by February 1st (October 1st for Spring Nursing). Please review the complete Instructions for Health Professions applicants.
Things to remember:
- You may apply for only one program per semester.
- Proof of attendance at the Pre-Admissions Orientation Sessions and program specific observations are valid within three years of planned program start date. If you have already completed these requirements within the time limit, you do not need to download the orientation and observation forms. You will still need to submit the Admissions Eligibility form.
- All applicants for a Health Professions Program must meet the requirements listed below for consideration.
- Priority in admission to all Health Professions Programs is given to legal Orange County residents and those applicants who have attended SUNY Orange.
- Current enrollment is not required for consideration.
- Meeting the Admissions requirements for a Health Professions Program does not guarantee admission into the program due to a limited number of seats in each program.
- All admission requirements must be in and/or complete by the deadline date for priority consideration—February 1, 2012 (October 1, 2011 for Spring Nursing).

For formatting purposes, these forms are only available in .pdf form. Please type the information into the form, print, and then send to the Admissions Office. If your browser does not accommodate .pdf files, please call the Admissions Office at (845) 341-4030 and we will gladly mail you the forms. Please also contact us if you require a more accessible version of these forms.
